Re-enrollment Form for Next School Year 2013 -2014
Please fill the form below to ensure a place is reserved for your child next school year. By submitting this form parents agree to pay tuition requirements for each child enrolled in the 2013-2014 school year. Until reservation fees are paid by April 1, seats cannot be guaranteed for next school year. This form will serve as an agreement between Parents and LCS that reservation fees will be paid. Any student who does not pay the reservation deposit will not be guaranteed a place at LCS in 2013 - 2014 school year and their position may be filled by the next applicant in the wait pool. Fees may be paid by check to the LCS Business Office, or directly to any of the school’s banking accounts. If you will not be returning please click here for information on our withdrawal process.
